The purpose of this form is to capture your concerns about the child/young person. The form is to be completed by staff or volunteers as soon as concerns are raised. It is not your role to investigate concerns. 

Forms are to be completed within one working day of the concern being observed. By submitting this form you are sharing the information with the TTNZ Child Safeguarding Representatives (CSR). If you also need to share this information with the affiliated Table Tennis organisation please note this in the information box towards the end of the form, including all details of who needs to receive this information and TTNZ will ensure to action this.
